Subject: REQUEST FOR FINANCIAL ASSISTANCE :
It gives me great pleasure to approach you with a request for your invaluable financial support towards continued successful functioning of SAS Free Poly Clinic at village Perch, Dist. SAS Nagar.
   
  Preamble:
Sahibzada Free Poly Clinic was inaugurated by Lt Gen (Retd) JFR Jacob, PVSM, the then Governor Punjab and Administrator UT Chandigarh on 10th March, 2002 with the salient aim of rendering quality medical services to our poor and needy rural brethren, irrespective of their religion, colour, caste or creed. To start with, it had four rooms, a modern X-ray machine, ECG Machine, Computerized Lab and an Allopathic Dispensary. It provided totally free Gynecologist's & Physician's services on Mondays & Wednesdays respectively.
   
  Activities of The Trust Since Its Inception:
1. The trust has added two dental chairs with requisite accessories, a lavatory block catering for female and male users separately; langar for providing tea & snacks on occasion, to patients coming from distant villages ;a water cooler with Aqua Gd water purifier; a homeopathic dispensary

2. The trust has added  two Dental Chairs with requisite accessories;    a Lavatory Block catering for female and male users separately;  Langar  for providing tea & snacks on occasion, to   patients coming from  distant villages ; a Water Cooler with Aqua Guard Water Purifier; a Homeopathic Dispensary;  a Noiseless Generating Set to get over the erratic electric supply in the area, a Physiotherapy Centre and an Ambulance.
   
  3. Besides adding required accommodation for the above and sheds with requisite furniture for the waiting patients, a lot of improvement has been done to its buildings from the hygiene, sanitation & aesthetic point of view.
   
  4. The OPDs (at the Clinic) have been increased gradually from 2 to 5 days a week. On the 6th day and two more days, our Mobile Dispensary, with invaluable help of Secretary Red Cross Punjab, renders 'Service at the Door-step' to those who cannot commute to our Free Poly Clinic. By this way we have already covered Nine villages/colonies. See note below).

3. A Lady Doctor (Gynecologist) On Wednesdays
4. Physician On Thursdays
5. A homeopath Doctor On Fridays
6. A Physiotherapist On Tuesday, Wednesday & Thursday
7. A Dentist  

On Wednesday & Thursday


Note:- Besides the above, till early 2012, our mobile dispensary visited Villages Teera & Jhampur, alternately, on Saturdays and Nayagaon on Wednesday to render’ at the doorstep’ health care service, in Homeopathy, to our poor & needy brethren who could not commute to our Poly Clinic at Village Perch . However, from February 2012, our mobile dispensary started visiting village Nayagaon on Mondays and Village Khuda Alisher / Colonies close to 39 Sector Chandigarh on Saturdays. since some time,with the invaluable help of the Secretary Red Cross Punjab, we have been visiting Naya Gaon on Wednesdays, Village Manana/Palsora on Thursdays and Maloya on Saturdays. In due course we plan to expand this service further.
   
 

5. Children’s Camps:
Eleven camps have been held, the last SIX being Multispecialty.  Over 5600 children benefited. Up To Tenth Camp, we were issuing medicines (vit A & D Capsules, iron and folic acid tablets for 30 days and a deworming dose for each child) to the Hd of the school to ensure administration of the medicines through the class teachers. We have since been informed that these medicines are being issued by the Civil Suegeon. We take special pleasure in placing on record that Lord Almighty made us “Nimat” in getting a 10 yrs old child, son of a poor “Mazdoor”,suffering with a Hole in the heart for over 5yrs, successfully operated in Fortis Hospital Mohali.

   

6. Eye Camps:
Till date, organized Nineteen Eye Camps (Incl camps at Paras 5,10&11). Over 3300 patients were examined and given treatment and out of this over 765 persons were operated for cataract totally free at the PGI, J.P Eye Hospital Mohali, Sohana Eye Hospital, Dr Grewal’s Eye Institute and at Sangam Netralaya, Mohali. All the Patients were provided, where required, free camping facility including food, dark glasses (initially, not any more), suitable spectacles(not to all), medicines and after operation care.

   
  7. Acu Pressure Camps:
We held two camps, first one for 5 days and the second for 7 days. A total of 242 persons were given treatment and their wards were trained to render follow up treatment.
   
  8. Ladies Camps:
Seven camps(Incl camps at Paras 10&11) were held & over 500 ladies were examined/treated,where required. During the last Five camps ( two at Perch, one each at Bothgarh, Garaunha and Panchkula) nearly 150 ladies were put through mammography and colposcopy tests. 
   
  9. Special Camps:
Two such camps were held with 11 doctors ex PGIMER's ENT, Speech & Hearing Dept and Trust's homeopathic mobile dispensary rendering 'Service at the Door-step' to over 550 belonging to two villages. Patients requiring further follow up at the PGI were given appointments and Mobile Ph No of the concerned Dr.
   
  10. Multi Specialty Camp:
Following two camps were held with doctors from most specialties & benefited over 670 persons. 3 persons were issued hearing aids

  a). On 16/02/2013. 17 Drs, 6 from Pgi, 2 from Grewal Eye Inst (GEI), one each from Ivy Hosp(Ortho), Sahayta Cancer(Gyne) and Cardio from Free Disp Gurudwara Sec 8 & 6 from our Trust (2 Dentists, 1 Gyne and 3 Gen Physi) benefitted 342 Persons. One was given Hearing Aid Times Health Care, 37 Ladies were put through Mammography & Colposcopy Tests and 71 were Op for Cataract at GEI.

  b). On 28/11/2015. Our Trust Allopaths & Homeopaths, PGI’s Drs from ENT cum Speech & Hearing and Radiology Depts and a team each from Sangam Netralaya and Ritu Chaudhary’s Hearing & Speech Clinic benefitted 350 Persons. All were tested for BP & Diabetes, 15 Op for Cataract, For Mammography & DEXA test ladies were sent in lot to PGI in Pb Redcross Ambulance and 2 were given Hearing Aid.

   
  11. Socio Medical Camp:
a). Our first camp, organized in collaboration with the SAS Nagar Dist Administration  was a grand success. 821 poor & needy from 11 villages were serviced by doctors covering most specialties. 30 Ladies underwent Mammography and Colposcopy tests; 71 Operated for cataract at Grewal Eye Institute; Hearing Aids were provided to 5 Persons. Socio Part of the Camp was rendered by concerned Depts of Dist Administration by way of Gen Awareness Programs, Aam Aadmi Beema, Loans, Pension to below poverty line persons, widows / handicapped & aid for appliances / unemployment allowances for the handicapped.

b). A similar camp was held on 5th Nov 2014 at Village Perch. 449 persons benefited. A large number of them were serviced both by the Medical and Socio Service providers. 40 underwent Mammography and Colposcopy tests (M & C tests). 3 persons were given Hearing Aids. 50 were operated for cataract by Sangam Netralaya,110 were put through Dental procedures at SAS Free Poly Clinic on different days during the period Nov 2014-Jan 2015.
c). An other Camp was held 8th November 2015 at Village Gharuan in which over 300 persons benefited by both Medical and Socio Service Providers. 40 ladies underwent M & C Tests; 23 were operated for cataract by Sangam Netralaya; 100 underwent dental procedure in the local Govt Dispensary.

d). An other similar camp was held at Panchkula in collaboration with Dist administration on 29th November wherein 475 persons benefitted. In that20 were operated for cataract at Grewal Eye Institute; 52 were serviced by ENT and Gynae specialists of CMO p’kula and one person was given free hearing Aid by Ritu Chaudhary’s hearing and speech clinic.

e). Another Similar Socio med Camp was held on 12 November2017 at Mohali in collaboration with Distt Administration, Silver city Rotary Club, Area Councillor Mohali Municipal Corporation at S Hari Singh Kahlon School Indus AreaPh 8 B where 450 persons from Shahid Udham Singh Colony Benefitted.

f). A Similar Camp was held at Govt Dispensary Sector 16 Panchkula on 17 December 2017 in collaboration with Municipal Corporation Panchkula, Health Dept (Hry) Panchkula where over 680 persons from Rajiv and otherColonies of the area benefitted.

  13. FUTURE PLANS:
The trust has a Dream  to have the clinic functioning on all six days in a week providing a wholesome care to our poor and needy rural brethren, urgently acquiring an additional ambulance, keep upgrading all our equipment and facilities, as and when required, and provide increased at the Door-step health care service.
   
  14) The Dream of the Trust can be fulfilled only with the donors kind and generous financial assistance towards providing, maintaining, upgrading and running the above mentioned facilities and meeting the day to day expenses for the medicines, pay and allowances of the Physiotherapist, technicians and the administrative staff and maintenance of the Buildings.
( All our doctors, except the Physiotherapist, the trustees and the social workers are highly devoted Philanthropists rendering tireless service totally free.)
   
  DONATIONS:
15) Donations to the Trust are exempt from I Tax under Section 80 G of I Tax Act 1961.
